GUIDE, which stands for Guiding an Improved Dementia Experience, provides families with the care they deserve through trusted home care agencies. In partnership with PocketRN and Nevvon, families can now receive 24/7 access and support from a dedicated care team. They can also access caregiver education in 11 languages, up to 72 annual hours of Medicare-funded respite care for eligible families, and personalized care planning that helps them stay connected to physicians, specialists, and trusted community resources.
All of this is available at no cost to families and no cost to agencies.
GUIDE represents a new standard for dementia care. It helps families stay strong, stay connected, and stay together at home with real support in place.
