Medicare’s Guiding an Improved Dementia Experience model helps families access ongoing dementia support at home at no cost for eligible beneficiaries. Through GUIDE, families can receive coordinated care navigation, practical caregiver education, and respite for eligible caregivers. PocketRN participates in GUIDE as a provider of services and partners with home care agencies nationwide so families can benefit where life happens most.
This week, one of our nurses met with a wife caring for her husband. She shared that she used the GUIDE respite benefit yesterday to attend water aerobics and do something for herself. She came home feeling lighter and more able to show up with patience and care. She thanked the nurse for walking with her through the ups and downs and said, “I never realized how much help I really needed, and you are helping me through it.” Moments like this are why access to respite for eligible caregivers matters and why a steady nursing relationship can change how the day feels at home.
